Our Medical Social Services Department staff is available to
assist patients and their family members who may need help with advance
directives legal documents. To request assistance from our social workers, call (740) 592-9337 or
(740) 592-9368 Monday through Friday from 8 a.m. through 4 p.m.
- Living Will
A living will is a document that tells a person's doctor or other healthcare providers whether the patient wants
life-sustaining treatments or procedures administered if he or she is in a terminal condition or a permanently
unconscious state.
- Durable Power of Attorney
A durable power of attorney is a legal document that allows a person to appoint another person to make medical
decisions for him or her if he or she should become temporarily or permanently unable to make those decisions.
