available to all employees, as some relate to eligibility requirements such as length of service, full-time or
part-time status, and/or salaried or bargaining unit status. Most of these additional benefits are not calculated

BENEFITS FOR BARGAINING UNIT EMPLOYEES
Separate benefit packages are provided for Bargaining Unit and Salaried employees. Bargaining Unit benefits are
described below. Benefits for Salaried employees will be discussed during the interview process.
PERSONAL LEAVE TIME
Employees earn 4 hours of personal leave time for every 80 hours paid, a total of 13 days paid personal time per
year for full-time employees.
VACATION TIME
All permanent employees shall earn vacations with pay based upon their years of service and employment status.
HOLIDAY PAY
O'Bleness offers 8 paid holidays per year.
TUITION REIMBURSEMENT
O'Bleness pays 80 percent of the cost of tuition for full-time employees up to a maximum of $1,200 per year,
pro-rated for part-time employees. The course work must be directly related to the position in which the employee
is working or could reasonably expect to work within the hospital.
BSN REIMBURSEMENT
Registered Nurses working toward a BSN degree whose employment status is at least .8 FTE during the quarter of
enrollment are eligible for 100 percent tuition reimbursement up to a maximum of $2,400 per calendar year.
CERTIFICATION PAY FOR PROFESSIONAL/TECHNICAL POSITIONS ONLY
O'Bleness will pay a $.25 per hour certification payment for an employee who obtains certification from a nationally
recognized certifying agency in the area where the employee is working.
SPECIALTY PAY FOR NON-PROFESSIONAL/NON-TECHNICAL POSITIONS ONLY
O'Bleness will pay a $.15 per hour specialty pay to employees who obtain a specialty certification or license in a
specialty applicable to their primary employment duties from a recognized certifying or licensing agency (agreed
upon by the Union and Hospital).
HEALTH INSURANCE
The hospital shall pay the full cost of single coverage for all active full-time employees, and, in addition, shall pay 90% of the cost of the difference between single and family coverage for all eligible full-time employees who elect such coverage. The hospital will pay one-half of the cost of single coverage for all permanent part-time employees with an employment status of 40 but less than 64 hours per pay period and three-quarters of the cost of single coverage for all permanent part-time employees with an employment status of 64 or more hours per pay period.
HEALTH INSURANCE WAIVER BONUS
Eligible employees may opt to waive their health insurance coverage and receive an annual bonus.
HEALTH & WELFARE PACKAGE
The coverage includes insurance for dental (including orthodontic for employee's dependent children under age 19), life, vision, hearing, and prescription drugs. O'Bleness pays the full cost of this benefit. This benefit is available for employees whose employment status is 56 or more hours per pay period.
CANCER/INTENSIVE CARE INSURANCE/SHORT-TERM DISABILITY
Employees may sign up to get group rates for individual or family coverage. Employees may choose Cancer Insurance,
Intensive Care Insurance, Short-Term Disability or a combination of the three. Cost varies depending on amount of coverage chosen.
ATTENDANCE BONUS
Employees who have worked the full payroll year and have completed their probationary period will be eligible for an incentive bonus ranging from $150.00 to $600.00 depending upon their FTE status and amount of unscheduled time off for the payroll year.
RETIREMENT 403B - TAX SHELTERED ANNUITY
The hospital will contribute a percentage of each covered employee's base wages determined at the time of his enrollment to the tax sheltered annuity program. The employee must be at least 21 years of age, worked at O'Bleness for 2 years, and worked 1000 hours or more each of those years to be eligible. In addition to the hospital contribution, covered employees may make additional contributions to their TSA account by authorized payroll deduction, to the extent permitted by law.
SHIFT DIFFERENTIAL
Employees working evening shift earn $.60 per hour in addition to their normal wages. Those working night shift
earn an additional $1.10 per hour.
CREDIT UNION
O'Bleness employees may have deductions made from their pay checks and paid to the Hocking Valley Credit Union to
pay off a loan or to be deposited into a savings account or Christmas Club.
CAFETERIA DISCOUNT
Employees receive a 10% discount on all meals purchased in our cafeteria while on duty.
OTHER BENEFITS
O'Bleness Memorial Hospital also offers several other benefits to its employees, including flu vaccine, blood pressure checks, colon-rectal cancer screening, height and weight checks. If the employee is
hospitalized, a private room for the cost of a semi-private is available.
SERVICE AWARDS
O'Bleness Memorial Hospital employees are formally recognized at each five-year milestone of employment. The Service
Award program includes service pins, gift selection and a semi-annual recognition luncheon.
